Elite 360 Innova Healthcare operates in New Jersey and provides medical and aesthetic treatments, including but not limited to consultations, skin resurfacing, wellness injections, and skincare products.

2. Appointments and Cancellations

• Scheduling: Appointments must be scheduled in advance via phone, email, or our online booking platform.

• Cancellations: We require at least 24 hours' notice for cancellations or rescheduling. Failure to provide notice may result in a cancellation fee.

• Late Arrivals: Arriving more than 15 minutes late may result in a shortened appointment or rescheduling at our discretion.

 

 3. Payment Policy

• Accepted Payments: We accept cash, major credit/debit cards, and approved financing options.

• Deposits: Some treatments may require a deposit to secure your appointment. Deposits are non-refundable unless otherwise specified.

• Refunds: Payments for completed services are non-refundable. In the event of dissatisfaction, we will work with you to address concerns within the scope of our practice.

 

4. Health and Safety

• Client Responsibility: You agree to provide accurate and complete medical history, including any conditions or allergies that may affect your treatment.

• Pre-Treatment Guidelines: It is your responsibility to follow pre-treatment and post-treatment instructions provided by our team to ensure optimal results and minimize side effects.

• COVID-19 Precautions: You agree to comply with any health and safety protocols in place, including mask-wearing and social distancing, when applicable.

 

5. Medical Disclaimer

• All treatments and consultations provided by Elite 360 Innova Healthcare are for aesthetic and wellness purposes and are not intended to replace medical advice or treatment from your primary healthcare provider.

• Results may vary from person to person. No specific outcome is guaranteed.

 

6. Product Sales

• Skincare Products: Products purchased from Elite 360 Innova Healthcare are final sale and cannot be returned unless defective.

• Product Use: Please follow usage instructions provided with skincare products. We are not liable for any adverse effects resulting from improper use.

 

7. Liability Waiver

By using our services, you acknowledge that certain treatments may carry risks, including but not limited to redness, swelling, bruising, or allergic reactions. You voluntarily agree to assume all associated risks. Elite 360 Innova Healthcare and its staff are not liable for any adverse reactions resulting from your treatment.
